• Send your ID and place your order with your email ending in .mil
• Service Record DD Form 180
• Certificate of Military Service
• DD214 Form for honorable discharges

 

HEALTHCARE WORKER 

• Badge/ID and number
• Position and name of institution

 

FIRST RESPONDER

• License or ID that includes your name and job title/position
• Recent training certificate
• Documentation that verifies your title and your name
